Diaper Bank Client Sheet - English

  1. Clients must complete this Client Information Sheet.

  2. Clients must show proper ID for the parent(s) and children who will receive diapers.

  3. Clients must show proof of Hall County residence.

  4. Clients must show participation in one of these programs:

    a. WIC, SNAP, or Medicaid.

    b. Program must include the name of the child or Birth Certificates must be shown.
